Following the “Pathways Community HUB” model, UniteMKE trains and develops Community Health Workers (CHW), who are trusted and acquainted service providers of the communities they serve. These CHW's utilize state-of-the-art data collection software to address the social determinants of health, to improve health outcomes. Through community partnership UniteMKE is the "Bridge" to assure optimal & efficient outcomes for both community members & service providers.
